Mildred Ocasio, LPN a licensed practical nurse has been practicing since 1997. With 20 years of experience as part of a primary care support team with many patient responsibilities.

As the Patient Care Coordinator at FHFHA, a patient centered medical home, she helps to coordinate care, and to be sure that her patients receive accessible high quality care within the current healthcare system.

Ms Ocasio continues to study and attends Rutgers University to obtain her BA in Public Administration.
